Manage plugins via the Tools menu, no restart required. (see bug 19118)

Known issues:
- A greyed out plugin means the parent directory is read-only.
- Windows-unfriendly: plugins in the registry are not managed at this time.
- Reload All will throw an exception if the plugin directories have not changed. Safe to ignore.
